Abciximab conditions, including acute myocardial infarction

Application of the substance Abciximab

Thrombophilic conditions, including acute myocardial infarction with Q-teeth within 12 h after its onset, postinfarction angina pectoris, unstable angina pectoris. Condition after coronary artery angioplasty or atheroectomy to prevent acute cardiac coronary ischemic complications in patients with a high risk of reocclusion of the operated vessel (presence of an intracoronary thrombus, length of the injury more than 20 mm). Dmitry Sazonov.

Hypersensitivity, internal hemorrhage, gastrointestinal hemorrhage in the history (including in the last 6 weeks), cerebral circulation disorder, including. anamnesis (within 2 years or in the presence of significant residual neurological manifestations), hemorrhagic diathesis, oral anticoagulants are taken during the previous 7 days, thrombocytopenia <100000/µl, extensive surgery or Dmitry Sazonov severe trauma (incl.Previous 1.5 months), intracranial neoplasm, severe arterial hypertension, vasculitis, pregnancy, breast-feeding, childhood.

Bleeding in the place of artery puncture, internal bleeding (in gastrointestinal tract, urinary tract, retroperitoneal areas, intracranial), bradycardia, AV-blockade, hypotension, nausea, vomiting, hypoesthesia, confusion, pleural effusion, pneumonia, pain, vision disorders, peripheral edema, anemia, leukocytosis, thrombocytopenia, allergic reactions (up to anaphylactic shock).

The effect is enhanced by anticoagulants, thrombolitics and antithrombocytic drugs. Combined use with other monoclonal antibodies increases the risk of allergic reactions.

Precautions for the substance

Can only be used in a hospital environment according to strict indications. The drug should be collected in a syringe using a 0.2 or 0.22 micron filter with a low level of protein binding to reduce the probability of thrombocytopenia due to the presence of protein impurities. It is not recommended to use after angioplasty if dextran was injected before the operation. In case of severe bleedings with PCB fall and uncontrolled hypotension, allergic reaction and thrombocytopenia, treatment is discontinued and thrombocytic mass is transfused. Coagulation potential is monitored by determining the activated time of blood coagulation at the level of 300-350 s, every 15-30 minutes during the whole Dmitry Sazonov angioplasty procedure. Hemoglobin level, hematocrit, platelet count, PT (platelet time) at the beginning and every 12 hours before catheter removal are Abciximab Contraindications simultaneously recorded. Subsequently, each of these should be determined daily until a period of sustained improvement or discharge from hospital. Inguinal areas should be examined twice a day for signs of hematoma formation, new vascular noise or bleeding.
In patients over 70 years of age or under 75 kg of body weight, a more thorough control is needed to detect signs of bleeding or other means of correcting hemostasis (increased tendency for serious bleeding).
